Patient Centered Health Care

The Patient Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means build a reliable primary health care system that delivers accessible, affordable and high-quality health care.

The medical home model is built on four general concepts and attributes for medical practice. It is Patient-Centered. The primary care medical home is oriented to the whole person emphasizing a partnership with the health care provider, the patient and their family respecting the patient’s unique needs, culture, values and preferences while actively supporting the patient’s ability to learn about and manage their own health care.

The Patient Centered Medical home provides Comprehensive and Coordinated Care. PCMH strives to meet the majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care and chronic care. This comprehensive care will be coordinated by a “team” of health care staff which will include physicians, advanced practice nurses and physician assistants, nurses, medical assistants, care managers, educators, social workers and the entire office staff working together to enhance the care and service within the office and community resulting in improved clear and open communication among patients and families, the medical home and the members of the broader care team.

PCMH provides for Superb Access to Care. The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced office hours and around the clock access by telephone to members of the health care team. The medical home practice is responsive to patient preferences regarding access.

Patient Centered Medical Home is a Systems-Based Approach to Quality and Safety. The primary care medical home is committed to quality care and quality improvement by ongoing utilization of evidence-based medicine and the use of tools and support guides to improve shared decision making with patients and families and the regular use of practice measurements and monitoring to improve patient care and outcomes while also measuring and responding to patient experiences and satisfaction.
St. George Medical Clinic has been committed to providing high quality medical care combined with dedicated medical care staff utilizing state of the art diagnostic equipment including electronic medical records. St. George Medical Clinic has chosen to dedicate significant efforts of our staff and providers to achieving full implementation of this concept and obtain National Committee for Quality Assurance (NCQA) recognition as a Patient Centered Medical Home because we feel that it is a critical component of our goal to achieve the full potential of primary care to impact and optimize disease management and the overall health of our community and patients.
